Semaglutide: The Deepest Evidence Base On This Desk

An FDA-approved GLP-1 receptor agonist backed by more large randomized trials, across more indications, than any other compound covered here.

The short version

Semaglutide is a GLP-1 receptor agonist — a synthetic peptide that mimics a gut hormone the body releases after eating. It is the lead compound on this desk because its trial record is the largest and broadest of the four: FDA-approved for type 2 diabetes, chronic weight management, reducing cardiovascular events in adults with established heart disease, and (since 2025) a form of fatty liver disease called MASH. It is available as a once-weekly injection and a once-daily oral tablet.

In the STEP 1 trial, weekly semaglutide produced a mean 14.9% body-weight reduction at 68 weeks versus 2.4% for placebo [19]. In SELECT, across more than 17,600 people with heart disease, it cut major cardiovascular events by 20% [18]. In FLOW, it reduced serious kidney-disease events by 24% in people with type 2 diabetes and chronic kidney disease [17]. The honest context: in a head-to-head trial, tirzepatide — a newer dual-receptor compound — outperformed semaglutide on weight loss by about 6.5 percentage points [16].

What it is

Semaglutide is a 31-amino-acid synthetic analogue of human GLP-1, about 94% identical to the natural hormone. Two changes give it its defining property — a long half-life. One substitution blocks the enzyme that normally breaks GLP-1 down within minutes; a fatty side chain attached elsewhere causes it to bind reversibly to albumin, the most common protein in blood, which shields it from being cleared and stretches its effective half-life to roughly a week. That is the entire structural basis for once-weekly dosing.

The oral tablet version is co-formulated with an absorption enhancer that lets a small fraction of the dose cross the stomach lining before it is broken down — oral bioavailability is still under 1%, which is why the tablet has to be taken on an empty stomach with minimal water.

How it works

Semaglutide activates GLP-1 receptors throughout the body. In the pancreas, it boosts insulin release only when blood sugar is already elevated and suppresses glucagon — a glucose-dependent mechanism that limits the risk of dangerously low blood sugar when it is used alone. It also slows how fast the stomach empties, which extends fullness after a meal and is part of why nausea is a common early side effect.

The weight effect is mostly a brain effect, not a metabolic one. Semaglutide reaches appetite circuits in the hypothalamus and brainstem, activating neurons that signal fullness and suppressing the ones that drive hunger — reducing both how much someone eats and the background mental preoccupation with food that people commonly call "food noise" [21]. Beyond appetite and glucose, GLP-1 receptors in the heart and kidneys appear to drive separate protective effects, confirmed in large outcome trials [17][18].

What the research shows

Head-to-head against tirzepatide. In a 72-week trial of 751 adults with obesity, tirzepatide produced significantly greater weight loss than semaglutide — 20.2% versus 13.7% (P<0.001) — making semaglutide the benchmark a newer dual-receptor compound has since surpassed [16].

Weight management (STEP 1). In 1,961 adults with overweight or obesity, weekly semaglutide 2.4 mg produced a mean 14.9% body-weight reduction at 68 weeks, versus 2.4% for placebo [19].

Cardiovascular outcomes (SELECT). In 17,604 adults with established cardiovascular disease and overweight or obesity but no diabetes, semaglutide reduced the combined rate of cardiovascular death, heart attack, or stroke by 20% relative to placebo (HR 0.80) [18].

Kidney outcomes (FLOW). In 3,533 adults with type 2 diabetes and chronic kidney disease, semaglutide reduced serious kidney-disease events by 24% relative to placebo (HR 0.76) [17].

Cardiovascular outcomes, type 2 diabetes (SUSTAIN-6). In 3,297 adults with type 2 diabetes at elevated cardiovascular risk, semaglutide reduced major cardiovascular events (HR 0.74) — though diabetic retinopathy complications were significantly more frequent in the semaglutide group (HR 1.76), tied to how fast blood sugar was corrected [22].

Mechanism, in rodents. Semaglutide's brain-circuit route to weight loss — reduced food intake and altered food preference without lowering energy expenditure — was mapped directly in mice and rats [21].

Safety synthesis. A dedicated review concludes the risk-benefit balance is favorable overall, with GI intolerance the dominant issue and unconfirmed pancreatic and thyroid cancer signals owing to low incidence [20].

Reported effects, cautions & safety

Patient-review sites and research communities describe a consistent pattern. These accounts are anecdotal, not clinical evidence — drawn from drug-review sites and forums, not controlled studies.

Reported benefits: a quieting of food-related thoughts within the first week or two; sharply reduced cravings for sugar and fried food; steady weight loss over months, usually tied to simply eating less; and, in people with diabetes, improved blood-sugar readings. A widely discussed secondary effect is reduced interest in drinking alcohol.

Reported downsides: nausea is the dominant complaint, worst in the first weeks and after dose increases; distinctive sulfur-smelling burps come up more often than official lists suggest; constipation and diarrhea, sometimes alternating; acid reflux; early fatigue on injection day; and, in a smaller group, hair shedding tied to the pace of weight loss rather than the drug itself.

What the clinical record actually flags: GI intolerance during dose escalation is the leading cause of discontinuation, and it is the drug's mechanism at work, not a rare reaction [20]. A boxed warning for thyroid C-cell tumors comes from rodent data; no confirmed human signal exists, but a personal or family history of medullary thyroid cancer or MEN-2 is a contraindication [20]. Gallbladder disease risk is genuinely increased, mostly attributed to the pace of weight loss itself [20]. Rapid blood-sugar correction can worsen existing diabetic retinopathy [20][22]. Substantial weight regain is documented after stopping — this is framed as chronic therapy, not a cure.
